The responsibilities of home health nurses are varied. Performs initial evaluation and conduct the re-evaluation of patients as required. Develop a plan of care in concert with the physician. Provide specialized nursing skills to meet the needs of the patient. Provide preventative/rehabilitative nursing measures. Prepares clinical records and maintain them. Coordinate home care services with all disciplines involved. Communicates with the physician, staff, patient, and family as appropriate. Counsel patient and family as required. Participate in in-services, supervise/instruct/evaluate home care personnel as required. Collect information at intervals outlined by Federal regulation. Utilized Evidence-Based Practice as a guide for clinical decision making and act as a case manager for the patients assigned by a supervisor.
Medical Social Services
Medical Social Service provides specialized counseling and support activities to the patient and family afflicted by illness. Therapy is rendered with the goal of achieving maximum psycho-social functioning within the family environment. Medical Social Services are ordered by and included in a Plan of Treatment authorized by the patient's physician. Service is usually limited to two to three visits with re-evaluation as needed. Physical Therapy services are ordered by and included in a Plan of Treatment authorized by the patient's physician. Therapy is ordered for a specific medical condition and must be restorative in nature. It is provided when there is an expectation that the patient's condition will improve in a reasonable amount of time. An evaluation for therapy and frequency of visits is determined by the patient's diagnosis, physical limitations and past condition. Physical therapy services include but are not limited to:
Occupational Therapy is a health profession that provides specialized treatments to assist patients to lead an independent, productive and satisfying life. Therapy is geared toward increasing the level of function for activities of daily living. If the level of function is limited by a disease process, the goal of therapy is adaptation and relearning of basic skills necessary for daily living. Occupational Therapy services are ordered by and included in the Plan of Treatment authorized by the patient's physician. Therapy is ordered for a specific illness or injury and concerned with improving or restoring functions. It is provided when there is an expectation that the patient's condition will improve in a reasonable amount of time. Specific Occupational Therapy services include, but are not limited to:
Home Health Aide
Home Health Aides provide assigned patient care under the direction of a registered nurse. Assisting the nurse in caring out the patient's plan of treatment by providing personal care to the patient as assigned by the nurse. Home Health Aide services are ordered by and included in a Plan of Treatment authorized by the patient's physician. Duties of a Home Health Aide can include but not limited to:
